Manual continuations.

It sounds like you would like to have the choices (choose) displayed. It looks like a combination of stop/resume commands and pick lists will give you exactly what you are looking for, stop commands to allow for the input of specific values and pick lists for your "choose" options.

Example:{command}Visual acuity was {Stop} OD and {Stop} OS/ {Stop} on both sides {QuickPick}uncorrected
with her usual glasses
with his usual glasses
with contact lenses{/QuickPick}.

You could also use a regular pick list for your choices if you wanted to use the same choices in more than one expansion.
