In Word: First, starting at the left margin, type a heading, e.g.,
Review of systems, followed by a space. Leave your cursor there.

Then ALT-T, Macros, Record.

In the Description box, highlight and delete what's there and put in the description of what you intend the macro to do and the keystrokes that will activate it. E.g., MAKE CAPITALIZED HEADING (CTRL-J).

Click on the Keyboard button.
