For instance my client ends every letter with "Thank you for allowing me to participate in your patient's care." I type "tyy;" and SOMETIMES I get "T hank you for..." This is really frustrating, but I don't have a clue why it is happening.

This does not seem to be the same issue as what JCG is referring to: In your case the capital does not disappear. (I am very much leaning towards the shift and space inversion in her case.)

For what you are describing, I suspect you are using WP8. Are you? We've found it to be one of the slowest word processors and suject to this kind of occasional inversions under stress. WP9 is much faster and we have never seen this happen with it. In any case, we are going to use a slightly different technique in a forthcoming revision which will better accomodate WP8 slowness.
