IT commands

I have spent all day on these commands. I wanted to put in

PMH: (control + F9)(end).

Allergies: (control + F9)(end).

tobacco: (control + F9)(end).

Fam. hx: (control + f9)(end).

ROS: ***NO COMMANDS ENTERED in this paragraph ** blah, blah, blah)

PE: This is a healthy, thin (control + F9)(end) looking stated age. Pulse(control + F9)(end). Wgt (control + F9)(end) etc.

IMPRESSION: (control + f9)(end)

It keeps cutting this body of text in half or flipping the last 3 items upside down, just plain mixing it up everywhere. What am I doing wrong. Could the paragraph without commands be mixing me up somehow. Also, is there a site to view how to use these commands, I have sent all day on this. 20 pages of dictation has taken me 10+ hours.

Any info appreciated.
