Suggestion Please

Would someone give me some suggestions on sentences such as this: He denies any shortness of breath, fever, chills, nausea, vomiting, or diarrhea.

I enter this as a phrase. Would you add the short forms with the commas included? I come across all variations of such sentences with different word ordering within the sentence. The doctor may say chills first in the sequence, etc.
