inserting copied text instead of shortcut

It happens sometimes with at four of my shortcuts. I actually wonder if it's something inside SoftPath. But I'm only a user not and IT tech.
I've even deleted the shortcuts (they had been imported from shorthand) and recreated them within Instant text. Still happens and only with 3 of my dictators. That's what weird about it. I type for more than a dozen doctors and its only happens with 3 of them with their cases.
Thanks for your suggestions but none of it actually helps me.
I don't want to copy/paste within the report, and I don't get why its pasting on its own, the last thing I copied from a different software instead of the shortcut I want to use.
