Let me give you an example. I use Quick Words to set up my template for my report. One of the headings is

DISPOSITION:

The text begins under the heading.

As I fill out the report, I down arrow to the various headings. Under DISPOSITION: I want to have - The patient tolerated the procedure well...

So, I down arrow to the line below the heading and press the Shift+Tpt for my expansion, letting loose of the shift after the first T. Sometimes it will expand as The patient, etc., and sometimes it will be the patient, etc.

I do not think it is caused by speed or getting my fingers mixed up while typing. It happens after a deliberate down arrowing to the next header, waiting for the dictation and then typing the expansion code using the Shift key for the first letter of that code. If I have typed a cap for that first letter and actually see it on the screen before moving to the next letter, sometimes it will lower case it and sometimes not.

Hopefully this is clear this time. It can be very frustrating to have to "Home" back to cap that first letter again.
